ALABAMA STATE DEPARTMENT OF EDUCATION DEFINITION OF VISUALLY IMPAIRED
I. Visually Impaired

1. Definition "Visually Impaired" means a visual impairment which even with correction adversely affects a child's educational performance. The term includes both partially seeing and blind. (see rules of Alabama State Department of Education, Division of Special Education Services (Special Programs Supp. No, 93-3, Ch. 290-080-090 p. 500).

II. D. PRIMARY READING MEDIUM: Indicate the reading medium for each student by checking the appropriate medium (i.e. for an infant, preschool or kindergarten student who has not yet learned to read. check "prereader", for a student who has no potential reading ability, check "nonreader").
II. E. GRADE LEVEL: Enter the following grade level codes only:
IP - For a student age 0 to 36 months to an infant program
PS - For a student age 37 months to 5 years in preschool program
KG - For a student in a kindergarten program which includes reading readiness, but is not the equivalent of first grade
01-12 - For a student in grade 1 through 12 (i.e, 08 indicate grade 8)
AN - For a student with normal intelligence who a. attends an academic program which does not assign conventional grade levels or which allows students to progress at an individual learning rate: or b. is a new student working to acquire the skills necessary for regular grade placement
PG - For a student in a post-graduate high school program, yet less than college level For a student of school age who is pursuing vocational training rather than academic studies
AD - For an adult in a rehabilitation center OR For a student who does not fall into the above listings
III. C. DEGREE OF VISION: Enter visual acuity for the right (O.D.) and left (O.S.) eyes separately. When possible, visual acuity reading should be given in terms of distance on the Snellen Chart. Even in the case of a non-communicative student, there must be verification that the student meets the criteria for eligibility, The following terms and abbreviations must be used.
20/200 - Snellen distance measurement (after best possible correction)
HM - Hand movements should be used only when an eye specialist finds it is not possible to obtain an acuity using the Snellen Chart.
CF - Counts fingers should be used only when any eye specialist finds it is not possible to obtain an acuity using the Snellen Chart
OP - Object perception
LP - Light Perception
NIL - Totally blind
III. D. PERIPHERAL FIELD: Enter this field (in degrees) if provided by the eye specialist
IV. AUTHORIZED PERSON: For public schools, the only individuals automatically authorized to sign this form are the System Coordinators of Special Education and their designees as specified in writing to the AIRCB. For other educational/rehabilitation programs and private schools, the administrator, principal or other chief officer may sign.
