How to Receive Outreach Services
There are only three steps to receiving AIDB Outreach services (for Alabama students with hearing and/or vision loss):
1. Access the Forms below (in English or Spanish) for the student's parent/guardian to complete.
2. Submit the completed packet to our Outreach department through email (outreach@aidb.org), FAX (256-761-3860), or post office mail.
3. Your assigned Outreach Provider will then contact you to discuss a service plan.
